iCON Chinese Health Forum
Healthy @ Home – Living Well with Chronic Conditions
The forum will bring together patients, families, health professionals, and community organizations to share information, self-management best practices, and community resources. Delivered in Cantonese with simultaneous interpretation in Mandarin and English, the forum will be available live online at www.iconproject.org, making this interactive education accessible to communities, patients and families across the province.

- 10:30am – 12:00pm Open House featuring health information booths, demonstrations, community resources and programs
- 12:00pm – 4:00pm Health Forum featuring panel discussions with community health experts
