Vancouver Coastal Health: Healthy Living - Diabetes – Provides information and services to prevent and manage diabetes.
Live Well with Diabetes Punjabi Video (Punjabi) – Video on Type 2 diabetes in Punjabi.
REACH and Multicultural Family Centre: Healthy Eating Active Living Program – A health education and health promotion program, focusing on developing long-term lifestyle changes to prevent and manage chronic disease and /or reduce the incidence of complications from chronic disease. Includes knowledge about and access to nutritious food, culturally relevant physical activity programs, and health information delivered in a culturally appropriate format.
Vancouver Island Health Authority - Adult Diabetes Education Centres – The Diabetes Education Centres provide education programs to adults with diabetes (and their families) to support them in managing diabetes.
Chronic Disease Management Program in British Columbia – The Chronic Disease Self-Management Program is a lay-led patient education program available across the province. Trained lay leaders meet with groups of 10-12 persons with chronic diseases for 2 1/2 hours once each week for six consecutive weeks. Course participants are persons who are experiencing any type of chronic health condition; their significant others are also encouraged to attend.
